Support Coordination
Support coordination
Someone in your corner, who knows the system
A support coordinator helps you understand your NDIS plan, find the right providers, and make it all work together – while keeping you in control of every decision.
The role
What a support coordinator does
The practical work, in plain terms. Every one of these is something we do alongside you – never over the top of you.
1. Make sense of your plan
We meet, talk through your goals, and help you understand what’s actually in your NDIS plan and what it can do.
3. Work with your whole team
Alongside your family, therapists and support workers, so everyone’s pulling toward the same goals – yours.
5. Keep an eye on your budget
We monitor your supports and spending with you, so the funding lasts the length of the plan – no nasty surprises.
7. Prepare you for reassessment
We gather the information and get you ready for your next plan review meeting, so you walk in prepared.
2. Connect you with providers
We help you make referrals under your Core and Capacity Building budgets – to providers who genuinely suit you.
4. Build your skills and knowledge
So you understand and can implement your plan yourself, as much as you want to, over time.
6. Handle the NDIA reporting
We complete the reports the NDIA asks for (usually around 8 weeks after implementation and at 9 months, depending on your plan).
8. Sort the bigger funding
We work with your providers to use capital funding — equipment, and home or car modifications — the way it’s meant to be used.
Why us
What makes us a little different
Plenty of providers can list the tasks above. Here’s what you don’t get everywhere.
We've lived it
Members of our team have lived experience of disability and of the NDIS. We understand the confusion and the difficulty of having a plan, because we’ve been there ourselves — not just read about it.
You stay at the centre
It’s your plan and your life. We keep you at the centre of every decision and in control of your own supports – our job is to make that easier, not to take it over.
We know it from the inside
Our team has worked for the NDIS – building plans and implementing supports. We know the NDIS Act, and we know where and when there’s room to be flexible with your supports.
No conflict of interest
Support coordination is the only thing we do. We don’t run the services we recommend, so the advice you get is genuinely about what’s right for you.
“We care, and we want the best outcomes for you.”
An honest heads-up about funding
Support coordination is sometimes a time-limited support, and it may not be funded in your next plan, depending on the level of support you need. If that’s the case for you, we’ll make sure you leave with a thorough understanding of how to carry on with confidence – because building your independence is the point, not keeping you dependent on us.
Two levels
Which kind of coordination is in your plan?
The NDIS funds coordination at different levels. Not sure which you have? Send us your plan and we’ll tell you.
Support Coordination
The everyday level — help to understand your plan, connect with providers, and build your confidence to manage supports over time. This is what most participants have.
Specialist Support Coordination
For when things are more complex — several services that need careful coordination, a support setup that keeps breaking down, or higher risk that needs managing. Funded separately in your plan.
