Making a referral

Whether you’re referring yourself, someone you love, or a client — this is the start of it. Fill in what you can, and we’ll take it from there.

Referring yourself

You have an NDIS plan, or you’re about to, and you’d like a coordinator. You don’t need anyone’s permission to refer yourself.

A family member or carer

You’re referring someone you support. We’ll want to know they’re happy for you to do it — see the note below.

An LAC, planner or hospital

We take referrals from planners, LACs and discharge teams. We’ll tell you honestly whether we have capacity.

Another provider

Referring a participant on, or looking for a coordinator to work alongside? Send it through.

Before you start

What’s handy to have nearby

None of this is compulsory. The more you can give us, the faster we can move — but an incomplete form is far better than no form.

  • The participant’s NDIS number, if you have it
  • Plan dates — when it started, when it’s up for reassessment
  • Whether the plan is self-managed, plan-managed or NDIA-managed
  • Which type of coordination is funded — Support Coordination, Specialist Support Coordination, or Recovery Coaching
  • A contact number and email, and the best way and time to make contact
  • Anything we should know to make first contact go well — communication preferences, access needs, things to avoid

Don’t have the plan in front of you? Send the form anyway. We’d rather start the conversation and chase the details than have you give up halfway through.

If you’re referring someone else: please make sure they know about it and are happy to go ahead. We’ll always confirm that with the participant directly before we do anything — it’s their plan, and their choice. If consent is complicated (guardianship, a nominee, someone in crisis), just tell us on the form and we’ll work it out with you.

    If we’ve got capacity, we’ll say so and get started. If we haven’t, we’ll say that too, and point you somewhere useful.
