The basics

What does an NDIS support coordinator do?

A support coordinator helps you understand and use your NDIS plan. We connect you with the right supports, set up service agreements, sort out problems when they come up, and help you build the skills and confidence to manage your own plan over time. The goal is to make the NDIS work for your life, not the other way around.

Do you provide support coordination in Bendigo and across the Loddon Mallee?

Yes. Your Path Your Way provides support coordination across the whole Loddon Mallee region, including Bendigo, Castlemaine, Kyneton, Gisborne, Woodend, Echuca, Heathcote, Maryborough, Kerang, Swan Hill and Mildura. Our team works locally and can meet you in the way that suits you best.

What's the difference between Support Coordination and Specialist Support Coordination?

Support Coordination helps you set up and use your plan. Specialist Support Coordination is for more complex situations, where barriers need a higher level of expertise to work through, such as several services that need to work together, risk that needs managing, or a crisis. Both are about building your capacity, not taking over.

What is a Psychosocial Recovery Coach, and how is that different?

A Psychosocial Recovery Coach supports people living with mental health challenges. Alongside the practical NDIS work, a recovery coach walks with you over time, using a recovery-focused approach to build routine, wellbeing and connection at your pace. It is a more relational role than support coordination, designed for the ups and downs that come with psychosocial disability.

Working with us

Is Your Path Your Way a registered NDIS provider?

Yes. We are a registered NDIS provider, registered for Support Coordination and Psychosocial Recovery Coaching (registration group 0106) and Specialist Support Coordination (registration group 0132). Registration means we are audited against the NDIS Practice Standards and work under the NDIS Code of Conduct.

Do you also provide other supports like SIL, therapy or plan management?

No, and that is deliberate. We only provide support coordination, specialist support coordination and psychosocial recovery coaching. Because we do not deliver accommodation, therapy, plan management or direct supports, we have no reason to steer you toward services we own. Our advice stays independent and conflict-free — we help you choose the providers that are right for you.

How do you make sure support coordination actually works?

We measure it. We track progress against your goals over time across the key areas of your life using our own outcome measurement system, so your progress is visible and based on evidence, not guesswork. It also means we can show you, and your planner, what is working and what needs to change.

Do you support people with complex needs or psychosocial disability?

Yes. Our Specialist Support Coordinators and Psychosocial Recovery Coaches are experienced with complex situations, including psychosocial disability, several services that need to work together, and times of crisis. We work in a neuroaffirming, trauma-aware way, and at a pace that feels safe for you.

Getting started

How do I know if support coordination is funded in my NDIS plan?

Look in the Capacity Building part of your plan for Support Coordination or Specialist Support Coordination. If you are not sure, your NDIS planner or Local Area Coordinator can tell you — or send us your plan and we will check it for you. There is no cost to ask.

What does support coordination cost me?

Nothing out of pocket. Support coordination is paid from the funding already set aside in your NDIS plan, at the rates in the NDIS Pricing Arrangements. You do not pay us directly, and we are always clear about how your hours are being used.

Can I change to a new support coordinator, and how?

Yes – you can change support coordinators at any time, and you do not need to give a reason. Just get in touch. We will handle a calm, organised handover so nothing falls through the gaps, and you stay in control of the pace.

How do I get started or refer someone?

You can refer yourself, or someone can refer with your consent. Send us a referral through our website, or get in touch by phone or email, and we will talk through your plan and how we can help. There is no obligation, and we are happy to answer questions first.

Why independence matters

Because support coordination is the only thing we do, we are never choosing between your interests and our own. That independence is written into how we work — and it is the difference that participants and families tell us they feel.
