Not happy with something? Let's put it in writing, together.

Answer the questions below in your own words. As you go, a complete letter builds on the right, ready to print, save or copy. There are no wrong answers, and you can leave anything blank if you're not sure.

You have the right to complain. Making a complaint won't affect your NDIS supports or the way you're treated. You can ask a support coordinator, a family member or an independent advocate to help you. If you'd rather talk it through first, your Your Path Your Way coordinator is happy to sit with you.

1 Who is the complaint going to?

Pick the one that fits best. Not sure? Your coordinator can help you choose, or start with the provider directly.

The letter will be addressed to the NDIS Quality and Safeguards Commission, PO Box 210, Penrith NSW 2751. You can also complain by phone on 1800 035 544 or through the online form at ndiscommission.gov.au. If you prefer, you can ask for your complaint to be confidential.
The letter will be addressed to the National Disability Insurance Agency, GPO Box 700, Canberra ACT 2601. You can also call the NDIA on 1800 800 110.
